Palliative Care Service Map

The Mississauga Halton Palliative Care Service Map is an interactive tool developed to provide health care professionals, patients and families with a listing of palliative care service providers. This map includes contact information and service boundaries.

Palliative care service providers included within this tool are LHIN Palliative Care Nurse Practitioners, LHIN Palliative Care Coordinators, Palliative Care Physician Groups, Palliative Care Service Provider Organizations, Visiting Hospice Counsellors/Coordinators, Residential Hospices, Palliative Pain and Symptom Management Consultants, NP STAT (listed under LTCHs), Long-Term Care Homes, Retirement Homes and Ontario Health Teams.
